Lash Service Questionnaire

Assumption of Risk: I understand that beauty services, including but not limited to lash extensions, facials, and skincare treatments, carry inherent risks, including possible allergic reactions, irritation, or sensitivities. I voluntarily assume all risks associated with these services.

Allergy & Reaction Disclaimer: I confirm that I have disclosed any known allergies or sensitivities. I understand that the service provider is not liable for any allergic reactions or adverse effects resulting from the service or products used.

Release of Liability: I release the service provider and Glam Lab PDX from any liability for any reaction, injury, or dissatisfaction resulting from the service. I waive any claims against the business or its staff for damages, injury, or loss.

Aftercare Responsibility: I understand that proper aftercare is my responsibility and that failure to follow recommended guidelines — including daily lash cleansing and refraining from facial products and makeup along both my upper and lower lash line — may affect my results.

Phone & Distraction Policy: I agree to put my phone on vibrate or silent mode during my appointment and refrain from taking non-urgent calls or meetings. If I anticipate needing to take an important call, I will communicate this in advance with the service provider.

Lash Density & Natural Lashes Requirement: I understand that achieving a very dense lash set requires a significant number of natural lashes. If I do not have the necessary natural lash volume, my set may not appear as dense as desired, and no refunds or adjustments will be granted based on natural lash limitations.
